Renal Consult

The Nephrology consult service at LAG-USC Medical Center consists of 2 teams, each with an attending physician, a fellow, 1-3 interns and/or senior residents, and sometimes a medical student. Consults are accepted at any time from medicine teams, non-medicine teams, and the ED

  • The Nephrology Consult Teams are expected to arrive by 7:30 AM on weekdays. The residents and medical students on the team will be overseen and led by the Nephrology Consult Fellow listed on Amion. (Amion.com, log-in: la gen, click on IM-Nephrology to see the daily schedule

  • ·       The nephrology consult team is expected to arrive by 7:30am. The resident and student on the team will be overseen and led by the Nephrology fellow listed on Amion. The resident, student and fellow pre-round on existing patients from 7:30am to 9:30am. The nephrology attending begins rounds at 9:30am. Rounds will typically focus on old patients and new consults from overnight. Generally, rounds first begin with patients in the ICU. Nephrology attendings are expected to perform bedside teaching, discussion of pathophysiology and current literature to aid in diagnostic and therapeutic decisions. The attending can also be found listed on Amion.

    • Every Monday at Noon the team attends core curriculum lecture which is 12 to 1pm.

    • Every Wednesday from 12 to 1pm is renal grand rounds and every Friday from 12-1p is clinical case conference.

    • In the afternoon the nephrology team will continue with patient care, take new consults and staff new consults with the Attending.

  • The nephrology consult team receives inpatient consultations throughout the work day from 7:30 AM to 5:00 PM. Consults are called to the Renal fellow VOIP. There are 2 renal fellow VOIPs, one for each team. Team A VOIP is x93996 and Team B VOIP is x93997. The nephrology consult team can also be reached via Teams consult chat.  

    • Lectures and didactics take place every Monday, Wed and Fridays from 12 to 1pm in the MMR 608 conference room or via Zoom

    • There is Core Curriculum: Mondays, 12:00 PM – 1:00 PM, in-person MMR 608

    • Renal Grand Rounds: Wednesdays, 12:00 PM – 1:00 PM, hybrid: Zoom, MMR 608

    • Clinical Conference / Journal Club: Fridays, 12 PM – 1:00 PM, Zoom

    • Every Friday at 8am is Campese rounds, where each team presents one case to Dr. Campese for discussion.

    • Fellows have their continuity clinic every Tuesdays and Thursdays, 1:00 PM – 5:00 PM, Multispecialty Center Building A, OPD 5p1

  • An important first step for a consultant is to identify and document the specific reason for consultation. “Dialysis” or “high potassium” are not adequate reasons, whereas “acute kidney injury assessment and treatment” or “suspected glomerulonephritis” is a more appropriate consultation.

    Data gathering: In most circumstances, the following are critical data elements to obtain and include when evaluation and presenting new consult patients: A thorough history of symptoms, onset and duration, laboratory testing such as urinalysis and microscopy, urine electrolytes and radiologic testing.

  • After rounds, nephrology consult team members are expected to communicate all recommendations to the primary teams via VoiP or Teams

    • All notes need to end with “Discussed with Nephrology Fellow and Attending Dr {name} and should be forwarded to the attending to addend and co-sign.

    • Remember that notes are legal documents, and you must take ownership of every sentence. Please do not copy and paste other team’s histories or exam findings.

    • Make sure you included subjective section which is separate from the hospital course. Make sure to document intake and output.

    • Please read your entire A&P and update as appropriate. If a differential is no longer being considered, then state so. If a diagnosis is made, please update it.

Fellows have Nephrology continuity clinic every Tuesdays and Thursdays, 1:00 PM – 5:00 PM in OPD 5p1

Residents rotating on the Nephrology service may be scheduled for clinic so please check your schedules.

Renal Clinic
