Night Float

The night float service consists of 7 different rotations: LAG medicine wards nights, LAG MICU nights, LAG CCU nights, Norris nights, Keck liver nights, Keck medicine nights, and Keck MICU nights. With the exception of LAG MICU and LAG CCU, senior residents cover the nights services (with an intern on LAC MICU/CCU). Each senior resident will have about 6 weeks of nights per year.

Important Information

    • Signout for all services is at 5:30pm, with the exception of Norris, Keck medicine, and Keck MICU which are at 6pm. In the am, signout is at 6:30, with the exception of Norris, Keck Medicine and Keck MICU which takes place at 7.

    • You should reach out to the day team prior to arriving on your first day to ensure you are added to any important teams group messages.

    • This service always begins on Tuesday night, and goes until the following Monday night.

    • LAG Medicine (Green Nights): Arrive at 5:30pm to accept signout from your 2 day teams. Patients can be assigned to you throughout the night in a round-robin fashion from prior to your arrival at 4:30pm to 4:30am. Should a patient be assigned to a day team after 4:30am, please assess and triage the patient and put in basic orders. You also must accept signout. You are responsible for any nursing calls for your two cross-covered teams and any patient you may admit through the night. An attending is available via phone for any questions.

    • LAG MICU: A senior resident and an intern (either ED, neuro, or medicine) will both arrive at 5:30pm to accept signout from their assigned team (either MICU A or MICU B). Throughout the night, the pair will be responsible for all calls and emergencies from their team they are covering, as well as any admissions or upgrades to the MICU. You will round with the fellow at some point early in the night to discuss a brief plan for each patient. A fellow is assigned to cover both MICU A and MICU B, and all admissions will go through the fellow. This fellow is also available at all times in house for questions and assistance.

    • LAG Cardiology: A senior resident and an intern will both arrive at 5:30 pm to accept signout from the Cardiology A and Cardiology B teams. Throughout the night, the pair is responsible for all admissions or upgrades to the Cardiology A and B teams, and for any nursing questions and calls for primary patients. At night the residents are not responsible for consults but do need to respond to Code STEMIs.

    • Norris: Arrive at 6:00pm to accept signout from the Norris Oncology, Norris Hematology, and Norris BMT (fellow-run service). Through the night you will be responsible for cross-covering all 3 of these teams and are the only physician in the building. An APP is available in-house to cover ICU level patients and respond to codes with you. A fellow is available via phone/pager at night.

    • Keck MICU: Arrive at 6pm for signout. You are responsible for cross covering the MICU, any admissions or upgrades, and you respond to any codes in the hospital throughout the night. A fellow is available in house. You will also round with the attending around 8:30pm to discuss the plan.

    • Keck Medicine: Arrive at 6pm for signout from Keck Medicine A and B. You will cross cover both these teams throughout the night. An attending is available in house and you will staff all new admissions with that attending.

    • Keck Liver: Arrive at 5:30pm to accept signout from the hepatology team. Throughout the night you will cross-cover all floor primary patients and admit all new patients whom often are coming as transfers from outside hospitals. You are also responsible for co-managing SICU hepatology patients with the SICU team in-house. A fellow is available via phone/pager.

    • Please arrive promptly for sign out. After signout, touch base with your fellow/attending (except on green nights) to establish expectations and discuss any potential admissions that may be arriving.

    • Please bring your pager to all Norris/Keck night float services as nurses may use this as a way of contacting you. It is also advisable to have Keck Teams set up on your phone.

    • You are responsible for all admission H&Ps for patients you admit throughout the night. All notes should be done prior to am signout and sent to the attending on service.
